What is Help Me Grow Brighter Futures’ Home Visiting Program?

Help Me Grow Brighter Futures’ Home Visiting programs provides parents and caregivers with personalized support and guidance during pregnancy and early parenthood at no cost. A professional trained in child development visits the home to provide  education, resources, and emotional support, focusing on maternal health, newborn care, and family well-being. 

  1. Nurse Family Partnership is a community home health program that provides a personal nurse to mothers throughout pregnancy until your child reaches age 2. The nurse will prepare you for childbirth, teach you about development, conduct screenings and empower you to access resources within your community. This program enrolls pregnant people up to 32 weeks. Income eligibility applies. 

  2. Healthy Families America provides support to expecting mothers to prepare for the birth and support new parents and caregivers with ongoing education on developmental milestones to ensure healthy development until your child reaches age 3. This program enrolls pregnant people and families with children up to 3 months of age. Income eligibility applies. 

  3. Parents as Teachers  provides parenting support and guidance on handling difficult situations, fun activities to boost development, conduct screenings, assist with achieving family goals, and connect you with community resources, playgroups, learning activities to ensure school readiness until your child reaches age 3. This program enrolls pregnant people and families with children up to 2 years of age. Income eligibility applies.

How do I apply for Home Visiting?

Families should fill out the referral form linked on the Mirza Eligibility Results Page or click here. An intake coordinator will contact you to determine which program best meets your needs and confirm your eligibility. If eligible, the intake coordinator will provide your information to program staff who will contact you to schedule an enrollment visit at a time convenient for you.

What documentation is required when I fill out my application?

If you are enrolled in Medicaid or Women Infants and Children (WIC), the intake coordinator will ask you for your ID numbers.

If you are pregnant, your home visitor may ask you for proof of pregnancy at your enrollment visit.  Examples of proof of pregnancy include an ultrasound picture with name and due date, lab test results, or screenshot of doctor’s notes or testing from an electronic medical records portal like MyChart.
